Most hair autografted hair transplants are performed in an office setting under local anesthesia. After thoroughly cleaning the scalp, the plastic surgeon will remove hair from a donor area somewhere else on your body. Ideally this is a portion of scalp from the back of the head. A portion of the hairy scalp is then removed with a scalpel and set aside while the surgeon sews the cut scalp closed.

After the scalp is sutured, small groups of hairs, or individual hairs are removed from the donor patch of scalp, as a hair transplantation. The area to receive the donated hairs is then also numbed with a cleaned and numbed with a topical anesthetic. Tiny holes are made with needles in the bald area and healthy hairs are delicately transplanted into holes. Candidates for Hair Transplantation

Hair replacement candidates must have healthy hair growth at the back and sides of the head to serve as donor areas. Donor areas are the places on the head from which grafts and flaps are taken. Other factors, such as hair color, texture and waviness or curliness may also affect the cosmetic result.

Transplant techniques, such as punch grafts, mini-grafts, micro-grafts, slit grafts, and strip grafts are generally performed on patients who desire a more modest change in hair fullness.

Indications for hair transplantation include androgenic alopecia, male pattern alopecia, cicatricial alopecia, traumatic alopecia, and traction alopecia.

Specific contraindications to this procedure include diffuse female pattern baldness, non–donor-dominant alopecia, and alopecia areata. The scarring alopecias are nondominant and, while active, do not respond to hair transplantation. These include discoid lupus erythematosus, lichen planopilaris, and other cicatricial alopecia.

An individual with very little hair might not be advised to undergo hair replacement surgery. Remember, there are limits to what can be accomplished.

There are four different types of abdominoplasties: the mini-abdominoplasty, standard abdominoplasty, extended Abdominoplasty and Circumferencial Abdominoplasty.These surgeries may involve liposuction. Skin and fat are cut away from the abdomen. The Mini-Abdominoplasty is the procedure of choice for women whose primary concern is a lower abdominal bulge but do not have much, if any, redundant skin. For such women whose bulge is due to laxity of underlying muscles and tissue, the Mini-Abdominoplasty is the procedure of choice. During this procedure a short incision is made at the level of the pubic hair and the abdominal muscles below the belly button are tightened creating a very flat appearance to the area.

A standard abdominoplasty procedure requires a much larger incision. Skin is separated from the abdominal wall, all the way up to the ribs. Muscles are sewn together to tighten them, and a significant amount of skin is removed, requiring the patient to be bent over for 10 to 14 days. Usually within 2 to 3 weeks the patient is able to resume normal activity, except for lifting. There should be no lifting or pushing, putting strain on the stomach muscles, for up to 2 months..

The extended abdominoplasty gives hope to the patient who is not a candidate for a standard abdominoplasty. The incision for the extended abdominoplasty goes to the back of the hip area. The incision low so that even low-rise clothing can be worn without scar exposure.

The circumferential body lift is a very aggressive procedure reserved for those patients with a significant amount of redundant skin posteriorly.

Brachioplasty (Arm Lift)

The effects of aging on the skin are familiar to all of us. Over time, the skin changes in four ways: it loses thickness, loses elasticity, loses adherence to the underlying tissue, and is affected by gravity. Retrognathia or micrognathism is a deficiency of the mandibular bone. This does not allow the upper and lower teeth to come together when chewing food and may affect speech. Sleep apnea may also be produced by a retruded or deficient lower jaw.

A deficiency of bone supporting the chin may require surgery (genioplasty) to build up the tissue and provide a normal framework for the chin. In this condition, also called hypoplasia, the growth of the maxilla does not match that of the lower jaw. There is a collapse of the normal mid-face supporting structures. In addition to causing difficulties with eating and speech, this deficiency may be associated with anomalies of the supporting structures of the nose and cheeks.

Procedures in Orthognatic Surgery

In orthognathic surgery, an osteotomy (bone cut) is made in the affected jaw, and the bones are repositioned in a more normal alignment. Generally, the bones are held in their new positions with plates, screws and wires. The patient may also need arch bars placed on both jaws to add stability (a procedure called intermaxillary fixation). For patients with deformities in both jaws, simultaneous osteotomies may be undertaken to achieve correction.

Patients with deficient bone or soft tissues of the face may require distraction osteogenesis. A distraction device is applied. Bone osteotomies are performed. The distraction device is then used to slowly apply a distractive force at the osteotomy site until new bone is formed. Once the desired reconstruction is achieved, the device is left in place until the bone is healed and then the device is removed. This enables a surgeon to elongate a facial bone and the adjacent soft tissue envelope. Patients with deficient bone tissue may require grafts from their ribs, hips or skull. Alloplastic replacement of missing bone may also be required. The preparation of a donor site and the harvesting of the patient’s own bone requires additional cognitive effort and surgical time on the part of the physician.

Orthognathic surgery or Jaw Surgery is the surgical correction of skeletal anomalies or malformations involving the mandible (lower jaw) or the maxilla (upper jaw).

These malformations may be present at birth, or they may become evident as the patient grows and develops. The more delicate cartilaginous tissues of the lower lateral cartilage generally require reinforcement with autogenous cartilage from the ear or septum to obtain a desired result. Affecting a result with pure cartilage reshaping techniques is difficult and usually inadequate. Septal cartilage frequently requires two-layered reinforcement because of its thinness.

Generally, the lower lateral cartilage is too soft and pliable to adequately support the tip. Such softness precludes the successful use of onlay grafts to the tip, except for the rare patient who has sufficiently strong cartilage (about 10% of patients). While septal cartilage appears adequate for tip projection in closed rhinoplasty, the compromise of circulation at the tip when using the open rhinoplasty approach may contribute to long-term weakness, absorption of the septal cartilage, or both. Advantages for placing the implants under the muscle include less interference with mammography and less rippling in the upper half of the breast. Generally, thin and small-breasted women should favor implant placement under the muscle. The advantages of less interference with mammography, less rippling of the upper half of the breast, and more cushion between the implant and the skin outweigh the drawbacks, many of which are temporary. Athletic women should consider placement of the implant above the muscle (as long as they have adequate breast tissue and body fat) to avoid breast distortion when the pectoral muscles are flexed. Thin, small-breasted, athletic women must weigh the options. In general, they are probably better off with implants under the muscle. Even though this can cause distortion of the breast while working out, this is not too much of a problem unless you work out in the nude. A more significant cosmetic problem for these women is rippling, which is less likely to occur in the upper half of the breast if the implants are placed under the muscle. Of course, this is your call. Be sure to discuss it carefully with your plastic surgeon. An alternative for these women is to have silicone breast implants.

According to some people, her huge boobs may be attributed to her weight gain. Sometimes when a woman gains weight, her boobs increase in size as well. A close up of her body in that area certainly reveals a full and copious C-cup chest. But the rest of her body has not really gotten fat at all. This weight gain explanation doesn’t add up.

Carmen Electra’s breast implants are in proportion to her body, as she has avoided trying to compete with the implants of fellow Baywatch co-star Pamela Anderson. Carmen’s breasts have been so popular that she has been featured in Playboy magazine four times. Carmen is mixed about her breast implants and confesses “I had nice breasts to begin with.
